Suggested order for 100%

Play the story through to Yaldabaoth. Talk to Leena and keep cooking at The Old Throne between stages so Golden and the meal achievements tick along on the way.
Max both meters (Focused, Omnismash) as currency allows. They make the later Underground levels far easier.
Sweep stages for the remaining Deevols, Scrolls and Pacts. Deevols open more Underground levels, so do this before the Underground push.
Clear all 20 Underground levels (Spelunker), then go back for no-damage clears (Untouchable).

21 on Steam. 9 are hidden, and all of those are boss kills.

What is the rarest achievement?

Untouchable (finish every Underground level without taking damage) and Rebellious Flame (unlock everything else) were both at 0.6% of players on October 1.

Are any achievements missable?

No achievement had been reported as missable in launch week. The game has three save files, so you can keep a separate save for experiments.

How long does 100% take?

The developer says up to 12 hours for a first playthrough and that getting every achievement "takes some time and effort" on top of that. Plan for the 20 Underground levels and the no-damage run to take the longest.
